ThisNP-PCCopened in August 2020,and we are currently attaching patients from the greater Victoria area for primary health care.The clinic follows the attributes of a Patient Medical Home, including:

• Processes to ensure all people in a community have access to quality primary care and are attachedto a primary care providerwithin their community

• 送货of extended hours of care, including mornings, evenings and weekends

• 送货of comprehensiveprimary careservices throughateam-basedcare approach.includingmanagement of common medical conditions as well as health promotion and prevention to patients

Be part of a cohesive team where each provider has their own unique role and scope in the provision of patient care, and you work together to make sure that thepatientneedsare met in the most effective way possible. Responsibilities would include:

• Referral to specialists
